We are a cash-based online clinic and health insurance is not accepted. We offer affordable, safe, and convenient care. Due to the nature of the service provided, refunds will not be given. 

 

Cancellation Policy

We request you give a 24 hour notice for a cancellation so that we may schedule others. If less than 24 hours is given you will be charged a $50 late cancellation fee rather than the quoted pricing at scheduling.

As a courtesy to others, if you are 15 minutes or more late you may be asked to reschedule. Patients who reschedule appointments less than 4 hours prior to the scheduled appointment will be charged a $50 fee for late rescheduling fee.

Patients who cannot attend their scheduled appointment must cancel or reschedule or contact Simply Lavish Health and Wellness to cancel or reschedule the appointment. If the patient does not show up for their appointment, it is considered a no-show, and the patient will be charged a $50 no-show fee. The patient will not be charged price quoted when scheduling.

 

To ensure patients get the appropriate care they need, Simply Lavish Health and Wellness ANP-C Nursing Corp may need to cancel or reschedule an appointment. If Simply Lavish Health and Wellness ANP-C Nursing Corp cancels or reschedules the appointment, the patient will not be charged a cancellation fee.
